Veterans Fact Sheets by Geographic Region

These information sheets were developed for use both inside and outside of VA. Inside of VA, they can be used independently or together by VA staff to explain to Veterans and families these important benefits. Outside of VA, they can be used by community hospice partners and others to help Veterans and families understand these benefits. The information sheet for palliative care may be especially helpful when a Veteran is not yet eligible for hospice care, and may still appreciate knowing that VA can address quality of life issues related to their serious illness (goals of care discussions, pain and symptom management).

VA Fact Sheet for Hospice Partners for the following areas:
New York: Dutchess, Ulster, Orange, Sullivan, Rockland, Putnam, Westchester Counties. New Jersey north of Newton, Sparta and Stockholm; parts of eastern Pennsylvania and parts of western Connecticut.

New York, Kings County, Queens County, and Richmond County. We also provide services to Veterans that reside outside our catchment area in the following counties: Bronx and Nassau County and areas in New Jersey.
